The weirdest enemy I've ever seen in a video game has to be Andrei Ulmeyda from Killer 7. When he gets injected with Heaven's Smile he goes insane and grows tentacles that lift his afro off of his head and he follows your through all these connected ambulances. He makes funny sounds when he attacks you and he is really slow. His weakness was just plain weird. You had to go behind him and shoot his afro. No, I'm not kidding, his afro is usually hanging behind him being dragged along by some of his tentacles and you have to get behind him and shoot it in order to kill him.

That's pretty damn weird, actually. XD I'm not sure I can top that at all, but.
I dunno if it's THE weirdest, but one of the weirdest enemies I've ever encountered was the boss called Cathedral in Xenosaga Episode II. It was... a cathedral. You're fighting a cathedral. It knows how to kick your ass, too.
After that you fight the space pope.

That big fish...toad...thing, in the Lakebed Temple in Twilight Princess was pretty weird.
The Nihilanth, the final boss in Half-Life was freaky, too. Giant floating baby that you kill by throwing explosives into its brain when its head opens up.

The final boss in opposing force was pretty much a tentacle monster, I'm glad this game wasn't developed in Japan or most of its attacks would probably involve rape.

Must agree to Pyramid head but the wierdest would be the giant baby in Zombies ate my Neighbours, freaked the hell out of me when i played it the first time.

Cathedral is the second-- wait, no, third to last boss in Xenosaga II, in the Omega System. So it's at the very end of the game.

There was some enemy in a Shadow Hearts game, I recall... it was an albino dude with a colossal penis. He would hit you with the penis as his attack. I still don't understand why I played that game. Maybe because of the opening cinematic. Guy in a train gets arm sliced off by demon. He picks up his arm, puts it back on, then proceeds to kick ass in an epic 3D train fight. Western appearance to the game early on, and I guess there was a gay wrestling vampire. Thanks to the person who helped me remember all this, you know who you are. War.
